Natural Health Services in Partnership with Tweed “Golden Ticket” Contest Rules and Regulations

  1. Natural Health Services (“NHS”) in partnership with Tweed Inc. (“Tweed”) “Golden Ticket” contest is open to all Canadians with the exception of Quebec, ages 18 and over, except representatives, employees and agents of NHS, and/or Tweed Inc., and persons with whom any of the aforementioned are domiciled.
  2. NHS in partnership with Tweed “Golden Ticket” contest commences at 7:00 a.m. MST on October 23, 2017, and ends once the Golden Ticket is found (“the Contest Period”).
  3. The Holder of the Golden Ticket, must redeem their ticket within six (6) months from the beginning of the Contest Period. The Golden Ticket must be presented in person at any NHS Education Centre or email Patientcare@naturalhealthservices.ca to identify they have found the Golden Ticket. In the event the Golden Ticket is found where a NHS Education Centre is not located; the finder must complete the “Contest Winner Form” and mail it along with the Golden Ticket and proof of identification to Suite 400, 355 4th Ave SW, Calgary, Alberta T2P 0J1. All risks of sending the ticket remain with the sender. To be declared a winner, the potential winner must hold a valid Golden Ticket and be 18 years of age. Before being awarded the prize, the potential winner will be required to sign a declaration and release form stating that he/she has read, understood and complied with these official contest rules, grants all consents required as contemplated in these Contest Rules, authorizes the Contest Sponsors to broadcast, publish and disseminate his/her name, address, photograph likeness and voice, in connection with any promotion or publicity, and/or for federal news, entertainment and information purposes at no compensation to the potential winner, accepts the prize as offered and releases the Contest Sponsors from any and all liability of any kind arising out of the potential winner’s participation in this contest and receipt and use of the prize. In the event that a potential winner is not eligible, or does not redeem the Golden Ticket, before contest close, NHS reserves the right to disqualify that potential winner and to draw an alternate potential winner and the Contest Sponsors shall be fully and completely released and discharged from any liability or responsibility in this.
  4. To enter NHS in partnership with Tweed Golden Ticket contest, simply visit any NHS Education Centre or www.naturalhealthservices.ca and purchase a Starter Kit. At the time of purchase, some of the prizes offered may have already been claimed.
  5. There will be one (1) Grand Prize to be won, consisting of: a personalized tour of Tweed facilities in Smith Falls, Ontario for two (2) adults, including flights, accommodations and a pre-paid credit card valued at $500.
  6. The Grand Prize has an approximate retail value of $2,500.00. Actual value of the Grand Prize will depend on current flight and accommodation rates. Winner and/or Guest is not entitled to monetary difference between actual prize value and stated approximate prize value, if any.
  7. Prize winner waives all recourse against Contest Sponsors if services offered do not prove to be entirely satisfactory.
  8. NHS will attempt to contact the potential winner(s) by telephone within five (5) business days after the ticket has been found and the winner has notified NHS. Proof of identification must be provided upon request. Before being awarded the prize, the potential winner will be required to sign a declaration and release form stating that he/she has read, understood and complied with these official contest rules, grants all consents required as contemplated in these Contest Rules, authorizes the Contest Sponsors to broadcast, publish and disseminate his/her name, address, photograph likeness and voice, in connection with any promotion or publicity, and/or for federal news, entertainment and information purposes at no compensation to the potential winner, accepts the prize as offered and releases the Contest Sponsors from any and all liability of any kind arising out of the potential winner’s participation in this contest and receipt and use of the prize. In the event that the potential winner is not eligible, or cannot be contacted by the Contest Sponsors within five (5) business days following the draw, does not sign and return the release within the specified time, Contest Sponsors shall have the right to disqualify that potential winner and to draw an alternate potential winner and the Contest Sponsors shall be fully and completely released and discharged from any liability or responsibility in this.
  9. The contest is subject to all applicable federal, provincial and local laws and regulations.
  10. Prize must be accepted as awarded without substitution. Prize is not transferable, cannot be redeemed for cash and cannot be sold, exchanged or bartered. Request for refund will not be entertained.
  11. The potential winner will be required to correctly answer a mathematical skill-testing question, without the assistance of any kind, whether mechanical or otherwise, to be administered during a pre-arranged time.
  12. NHS, Tweed and related Contest Sponsors are not responsible for theft, loss of any form or any human or computer-generated error in the awarding of the prize, nor for the loss or misplacing of any Golden Ticket.
